INSURANCE SCHEDULE

        BELANGER BROTHERS MAINTENANCE, INC.   will hold    
[your Company], its officers, agents and employees
harmless from liability of any nature or kind including costs and expenses for on account of any
law suit or damage claim of any character what-so-ever resulting from injuries by virtue of the
performance of this contract by BELANGER employees or its agents.

        The insurance required shall be carried with such underwriting companies as may be
approved by the management of   [your Company]  And
shall certify that the policy contains:
                                               

                                1.   Workmen’s Compensation Insurance as required
                                       by the laws of the State of Michigan.

                                2.    Public Liability Insurance for Bodily Injury
                                       $100,000 / $200,000

                                3.    Property Damage.................$1,000,000 / $2,000,000

                                4.    Automotive and Automotive Equipment Insurance:
                                       Combined single limit 1,000,000 Including hired & non owned.
 
                                5.    Umbrella Policy....................$2,000,000
